Required Photo copies of Operator/supervisor (PAN Card, Aadhaar, operator/supervisor certificate ) & Document of Verifier – (self attested aadhaar photo copy & PPO /any other document in support of retired govt. employee or Consent of serving govt. official when he willingly wants to perform verifier job in following format prescribed.
